TOP-BRKDN: Breakdown Repair

Scope: This procedure covers the maintenance of equipment other than scheduled preventive maintenance. Primarily this involves breakdown repair, but also covers any unscheduled maintenance that might be undertaken to avoid an imminent breakdown.

1 Any team member Equipment requires repair or unscheduled maintenance Own observations Report problem to supervisor.
2 Any supervisor or manager Equipment requires repair or unscheduled maintenance Own observations or reports from others Report problem to Maintenance Supervisor or Maintenance Leadman, or to Maintenance Worker.
3 Maintenance Supervisor , Maintenance Leadman or other receiving call Receive request for equipment repair Request for equipment repair; own knowledge of equipment and resources Determine priority of trouble call, who will handle it and when; if problem is not going to be addressed immediately, inform requester of priority and time frame; issue AMMS Work Orders to maintenance workers and/or verbal instructions
4 Maintenance Worker As assigned AMMS Work Order and/or verbal instructions Initiate AMMS Work Order (if work will take more than a few minutes); perform tasks as assigned; report to appropriate supervisor when complete; report any other work that is needed, any ideas for improving the equipment or its operation, or points that ought to be added to the equipment's PM AMMS Work Order (report to Maintenance Leadman or Maintenance Supervisor ); mark work done on AMMS Work Order and return it to Maintenance Supervisor; if furnace temperature controls in need of repair or calibration, inform Maintenance Supervisor .
5 Maintenance Supervisor Temporary repair is needed Own observations and evaluations; input from Production Supervisor Once a temporary repair is merited and completed, issue a AMMS Work Order; have the AMMS Work Order show the date the temporary repair is to be replaced by the correct permanent repair; put equipment back in service when repairs completed.
6 Maintenance Manager; VP-Manufacturing AMMS Work Orders for temporary repairs are past their due date AMMS Work Orders for temporary repairs Determine the reason a AMMS Work Order for a temporary repair was not completed on time; assign a new due date to the AMMS Work Order.
7a Maintenance Supervisor Furnace temperature controls are in need of repair or calibration Notification from Maintenance worker Issue AMMS Work Order; inform Calibrations Clerk via E-Mail to note on calibration log; QA Assistant will send instrument to approved calibration supplier.
7b Maintenance Supervisor Furnace temperature control fails calibration and cannot be economically repaired Results of calibration Tag instrument with reject label; notify Calibrations Clerk of final disposition of instrument via email.
7c Maintenance Supervisor Furnace temperature control passes calibration Results of calibration Notify Calibrations Clerk of calibration date and next calibration date via email; tag instrument with calibration label showing ID, calibration date, next calibration date and vendor name.
8 Maintenance Supervisor AMMS Work Orders returned AMMS Work Orders plus own observations or tests Review any suggestions for improvements; return completed AMMS Work Order to Warehouse Administrator to close work order.
9 Warehouse Administrator Weekly Completed AMMS Work Orders Perform any maintenance job costing requested by Maintenance Supervisor; enter info into AMMS; discard AMMS Work Orders.
10a Any supervisor or manager Requesting major changes of equipment or modifications; requesting additions of internal or external building structure Own observations and knowledge Submit a request to the Maintenance Engineer stating the requested changes; if Maintenance Engineer is unavailable, submit request to the Maintenance Manager.
10b Maintenance EngineerMaintenance Manager Upon receiving a request for major changes of equipment, equipment modification, or additions to building structure is received Request for changes; own knowledge of equipment and resources
Evaluate changes requested, verifying the proposed changes meet safety standards and codes; document approval in maintenance public folder and create plan of action for maintenance to accomplish changes.
10c Maintenance personnel After receiving approval, plan of action and AMMS Work Order Own observations and knowledge; Maintenance Engineer's plan of action Initiate modifications; perform tasks as assigned; report to appropriate supervisor and Maintenance Engineer when complete; mark work done on plan of action and return AMMS Work Order to Maintenance  Engineer.
10d Maintenance Engineer After plan of action is complete
Own observations and knowledge Verify work was done properly and will pass safety inspection; if approved, return to Warehouse Administrator (to enter then discard) completed AMMS Work Order with notes that completed modifications met safety standards and codes; place a copy of inspected AMMS Work Order in Maintenance Public Folder.
